According to my research, there are so many complications after Lasik. I had consulted with many eye doctors. None can sure what kind complications I will have after Lasik. They all said that it depends on individual. It made me more confused about Lasik surgery.

When LASIK complications occur, they may be associated with the creation of a hinged flap in the clear front covering of the eye (cornea), which is lifted for laser re-shaping of the eye. The flap then is replaced to form a type of natural bandage.

If the LASIK flap is not made correctly, it may fail to adhere correctly to the eye’s surface. The flap also might be cut too thinly or thickly. After the flap is placed back on the eye’s surface, it might begin to wrinkle. These microscopic wrinkles in the flap are called corneal striae.

These flap complications can lead to an irregularly shaped eye surface. Most such problems can be resolved through re-treatment of the eye with enhancement surgery.

Studies indicate that flap complications occur in from 0.3 percent to 5.7 percent of LASIK procedures, according to the April 2006 issue of American Journal of Ophthalmology. But inexperienced surgeons definitely contribute to the higher rates of flap complications. Again, remember that you can reduce your risk of LASIK complications by choosing a reputable, experienced eye surgeon.

Some problems associated with LASIK flap complications include:

Irregular astigmatism can result from a less than smooth corneal surface. Irregular astigmatism also can occur from laser correction that is not centered properly on the eye. Resulting symptoms may include double vision (diplopia) or “ghost images.” In these cases, the eye may need re-treatment or an enhancement.

Diffuse Lamellar Keratitis (DLK), nicknamed “Sands of the Sahara,” is inflammation under the LASIK flap that may have several causes. Some inflammation of the cornea after LASIK surgery is normal. But if it is uncontrolled, as in DLK, it can interfere with healing and cause vision loss. If DLK occurs, it usually responds to therapies such as antibiotics and topical steroids. The flap also might need to be lifted and rinsed for removal of inflammatory cells and to prevent tissue damage.

Keratectasia or keratoconus is bulging of the eye’s surface that can result from a flap that is cut too deeply, when too much tissue is removed from the cornea during LASIK or when the cornea initially was weakened as evidenced from cornea topography mapping prior to LASIK. Resulting distorted vision likely cannot be corrected with laser enhancement, and gas permeable contact lenses or corneal implants (Intacs) may be prescribed to hold the cornea in place.

A promising new treatment for keratectasia (also called “ectasia”) is corneal collagen cross-linking with riboflavin (C3-R). In this non-invasive procedure, eye drops containing riboflavin (vitamin B2) are placed on the cornea and then are activated with ultraviolet (UV) light. This strengthens the links between the collagen connective tissue fibers within the cornea to halt the bulging of the eye’s surface.

Other, more commonly reported complications that can result from LASIK eye surgery include:

Dry Eye After LASIK: Almost half of all patients reported problems with dry eyes after LASIK within the first six months, according to the April 2006 issue of American Journal of Ophthalmology. These complaints appear related to reduced sensitivity of the eye’s surface immediately following the procedure. If you have this problem, temporary remedies such as artificial tears or prescription dry eye medication may be needed along with oral flaxseed oil capsules.

Significant Undercorrection, Overcorrection, or Regression: This means that your outcome is less than optimal and makes it difficult to function in certain situations. There are various reasons for these problems. In many cases, the cause is simply that a patient’s eyes did not respond to laser eye surgery in a predictable fashion. But it’s also possible that results are related to an inaccurate diagnosis or incorrect settings programmed into software guiding the laser during eye surgery.Regression from “over-healing” occurs when your eyesight is optimal at first, but then begins to deteriorate over time. In most cases, a significant undercorrection or regression can be treated with additional laser vision correction or other refractive surgery methods such as conductive keratoplasty, once it is certain that the refractive error is stable.

Eye Infection or Irritation: These problems are rare and may require treatment with eye drops containing antibiotics or anti-inflammatory medication such as steroids. When bacterial infection occurs after LASIK, it appears to be related increasingly to an antibiotic-resistant form of Staphylococcus aureus. Survey results indicating a growing problem with this type of infection were reported at the 2008 annual meeting of ASCRS.LASIK surgeons also reported that infections occurred more frequently with surface ablations such as PRK and when bladed instruments called microkeratomes are used to create the flap during LASIK surgery. ASCRS reports indicate that flaps that are lifted and replaced on the eye during LASIK, serving as a type of “natural” bandage, appear to create a more sterile surgical environment than PRK and other surface ablation procedures.

I am thinking about if I am not a Lasik cadidate. What else can I do for my increasing numbers.

Even if you are not qualified for LASIK, you still might be able to undergo vision correction surgery through other means such as PRK, LASEK, or implantable lenses. If you do decide to have LASIK, a responsible eye surgeon will work closely with you to resolve problems if they do develop. You can choose a LASIK surgeon by taking certain steps such as checking credentials.

But eye surgery is after all an invasive treatment. No one can sure how long the surgery effect would last. I decided to try some innovative physical, non-invasive product such as EyeLoveYou restore vision Pioneer to save my eyes.

Your eyes must be healthy. If you have any condition that can affect how your eyes respond to surgery or heal afterwards, you must wait until that condition is resolved. Examples are chronic dry eyes, conjunctivitis and any injury. Some conditions, such as cataracts that interfere with your vision, keratoconus and uncontrolled glaucoma, may disqualify you completely.

You must be an adult. By law, certain procedures require you to be 18; others, 21. Younger patients can be treated as an exception.

You must have stable vision for at least a year. Many young adults experience changes in their eyeglass and contact lens prescriptions in the teenage years. Vision stabilizes most often sometime in their 20s. Usually, it is nearsightedness gradually becoming worse, but there may be other changes as well. They are not good candidates until their eyes have “settled down” into one prescription. Your doctor can tell you whether your prescription is stable.

If you are pregnant, certain hormonal changes will cause fluid levels in your body to rise. This can change the shape of your corneas, leading to changes in your vision. Surgery should not be performed until your hormones and vision have “normalized” after pregnancy. This could take a few months. Dry eyes are often seen in pregnant women as well, and as mentioned above, you should postpone LASIK until your eyes are healthier. In addition, some medications that would normally be used before or after surgery to promote healing (such as antibiotics and steroids) may be risky for your baby, whether unborn or nursing.

Degenerative or autoimmune diseases may be disqualifiers, too. Some examples are Sjogren’s syndrome, rheumatoid arthritis, type 1 diabetes, HIV, and AIDS. Basically, if your body has any trouble with healing, your refractive surgery outcome may not be very good. Opinions vary among LASIK surgeons as to which diseases are automatic disqualifiers and which ones might pose acceptable risks in certain cases.

Your prescription must be within certain limits. For example, very high amounts of myopia, which would require removal of too much corneal tissue, may preclude LASIK or make another type of refractive surgery a better option, such as insertion of artificial lenses known as phakic IOLs.

A comprehensive study reported in Archives of Ophthalmology in 1994 noted that 95.5 percent of healthy eyes achieved 20/40 uncorrected vision (legally acceptable for driving) or better outcomes following cataract surgery. Of the more than 17,000 eyes evaluated, fewer than 2 percent had sight-threatening complications.

Bruising or a black eye can result from cataract surgery, if an injection is used to numb the eye.

Remember that sight-threatening complications often are associated with individuals who are much older or who already have poor underlying health affecting how their eyes heal. Also, some people have complications because their cataracts are far more advanced or “hardened” at the time of surgery, making them difficult to remove.

A Swedish study published in the British Journal of Ophthalmology in November 1999 found that self-reported outcomes among people who had undergone cataract surgery were less satisfactory when other eye problems were present. Younger people undergoing cataract surgery reported the highest satisfaction levels.

The British journal also reported study results in December 2000 indicating that people in their 60s undergoing cataract surgery were 4.6 percent more likely to achieve 20/40 uncorrected vision or better than people in their 80s

My mother had cataract surgery one year ago. So far, she felt that the surgery was not too bad, but didn’t match her expectation. I did some research about the complications of Cataract Surgery.

Glaucoma or a buildup of pressure within the eye (intraocular pressure) also occurs sometimes after cataract surgery. If your eye pressure remains high, you may need additional treatment such as eye drops, a laser procedure, pills or additional surgery.

Phacoemulsification in cataract surgery involves insertion of a tiny, hollowed tip that uses high frequency (ultrasonic) vibrations to “break up” the eye’s cloudy lens (cataract). The same tip is used to suction out the lens.

After the eye’s natural lens is removed during cataract surgery, an artificial or intraocular lens is implanted to take its place.

Far more rarely, you might experience problems such as a decentered intraocular lens that might need to be repositioned or replaced in a second surgery.

Various complications, ranging from minor to serious, also can take place as a direct result of the surgical procedure, including tearing of the posterior capsule holding the intraocular lens in place.

Detached retinas also are possible in a small percentage of people who have undergone cataract surgery, particularly if they have unusually long eyes associated with higher degrees of nearsightedness.*

Some eye surgeons dispute this direct association with cataract surgery, because highly nearsighted people already are at risk of getting a detached retina with or without cataract surgery. Cumulative rates of detached retinas occurring in highly myopic general populations who underwent cataract surgery or refractive lens exchange are roughly 1 percent in some studies, which is about the same risk if you never underwent a procedure.

However, a common complication that creates a “secondary cataract” may require a YAG laser capsulotomy procedure. A high myope who undergoes both cataract surgery and a subsequent YAG laser capsulotomy may have a significantly greater risk of developing a detached retina.**

Endophthalmitis causing widespread inflammation or infection of the eye can be a serious side effect of cataract surgery that can lead to permanent vision loss and even blindness. Various studies indicate that endophthalmitis occurs in about one out of every thousand cataract surgeries. Endophthalmitis also is more likely to be seen in people with compromised immune systems associated with conditions such as diabetes.

However, even serious cataract surgery complications often can be resolved with appropriate follow-up treatments.

When my mom’s operation was over, the surgeon placed a protective shield over her eye. After a short stay in the outpatient recovery area, she was ready to go home. Be sure you need to plan to have someone else drive you home!!

You will need to administer eye drops, as prescribed by your surgeon, several times daily during the next few weeks. You also will need to wear your protective eye shield while sleeping or napping, for about a week after surgery. You will be given sun shades to help protect your eye in bright light.

During at least the first week of your recovery, it is essential that you avoid:

Strenuous activity and heavy lifting (nothing over 25 pounds).

Bending, exercising and similar activities that might stress your eye while it is healing.

Water that might splash into your eye and cause infection. Keep your eye closed while showering or bathing. Also, avoid swimming or hot tubs for at least two weeks.

Any activity (such as changing cat litter boxes) that would expose your healing eye to dust, grime or other infection-causing contaminants.

Although the basic postoperative instructions are similar among most eye surgeons, each surgeon may have specific recovery instructions depending on the outcome of your surgery. Always follow your surgeon’s specific instructions, which you will receive prior to your discharge from the outpatient facility.

Here are some things YOU can do to help make YOUR cataract surgery experience more successful.

1. Make sure your surgeon had addressed any co-existing eye conditions and how they will affect your outcome. Macular degeneration, macular pucker, diabetic retinopathy, glaucoma and dry eye keratopathy are the main co-existing conditions we see.

2. Make sure you let the surgeon and anesthesiologist know if you have had trouble with surgery or anesthesia before and if you are extremely nervous or have clostrophobia.

3. If you are feeling ill and having trouble with fevers or a cough or shortness of breath please let the surgeon know and be ready to possible delay the surgery.

4. If you have blepharitis or granulated eyelids follow instructions on using baby shampoo or lid cleaning pads like Ocusoft in addition to an antibiotic ointment for the prescribed time before surgery.

5. Make sure you are able to actually put in your eyedrops properly and follow the drop schedule exactly. If you can’t do it yourself you will need to have someone do it for you. This is one of the main areas where I run into trouble is when patients forget to use the drops or think that they are doing so well that they don’t need to use them.

6. After surgery report any problems right away to the surgeon’s office. All ophthalmologists will have a 24 hour call service that will relay your call to the doctor on call that day. Don’t be shy or afraid or embarrased to call your doctor. He or she needs to know right away if you’re having a problem.

7. Keep aware of your medical allergies and make sure that none of the drops or medications you are given are not on your allergy list. I know that is mainly the job of the doctors and nurses but mistakes can still happen and if you’re paying attention you could help to prevent a problem.

8. Stay clean, wear clean clothes, take a good shower the night before and wash well especially around your face. Wash your hands a lot and try to keep them away from your eyes if possible. These are just common sense things to help reduce the risk of infection.

Good Luck, and God Bless.

An intraocular lens (IOL) is implanted in the eye in place of the clouded natural lens. Shown is Alcon’s AcrySof Natural IOL; it filters out blue light, which may be harmful to eyes.

Think about surgery when your cataracts have progressed enough to seriously impair your vision and affect your daily life. Many people consider poor vision an inevitable fact of aging, but cataract surgery is a simple, relatively painless procedure to regain vision.

Cataract Surgery is very successful in restoring vision. In fact, it is the most frequently performed surgery in the United States, with more than 3 million Americans undergoing cataract surgery each year. Nine out of 10 people who have cataract surgery regain very good vision, somewhere between 20/20 and 20/40.

During surgery, the surgeon will remove your clouded lens and in most cases replace it with a clear, plastic intraocular lens (IOL).

New IOLs are being developed all the time to make the surgery less complicated for surgeons and the lenses more helpful to patients. Presbyopia- correcting IOLS potentially help you see at all distances, not just one. Another new type of IOL blocks both ultraviolet and blue light rays, which research indicates may damage the retina (see illustration).

My mom had suffered from Cataract for two years. In the beginning, she didn’t know that is Cataract. She felt panic about her more and more blurry vision for long time.

A cataract starts out small and at first has little effect on your vision. You may notice that your vision is blurred a little, like looking through a cloudy piece of glass or viewing an impressionist painting.

A cataract may make light from the sun or a lamp seem too bright or glaring. Or you may notice when you drive at night that the oncoming headlights cause more glare than before. Colors may not appear as bright as they once did.

The type of cataract you have will affect exactly which symptoms you experience and how soon they will occur. When a nuclear cataract first develops, it can bring about a temporary improvement in your near vision, called “second sight.” Unfortunately, the improved vision is short-lived and will disappear as the cataract worsens. On the other hand, a subcapsular cataract may not produce any symptoms until it’s well-developed.

If you think you have a cataract, see an eye doctor for an exam to find out for sure.
